This Privacy Policy covers website use only. Protected health information created or maintained for care is governed by our Notice of Privacy Practices.
Information you provide. Our contact form collects your name, email address, phone number, company name (if provided), and message. Please do not include medical details in the form; for health questions, call our office.
Information collected automatically. The Site may collect IP address, browser and device information, pages viewed, referral source, and campaign information through cookies and similar technologies.
We use essential cookies, including our cookie-consent cookie, Cloudflare security cookies, and form spam-protection cookies, to operate the Site, remember your cookie choices, and protect against fraud and spam. With your consent, we also use Google Tag Manager to manage Site tags, Google Analytics 4 to understand how the Site is used, and Google Ads to measure advertising performance. These tools may collect information such as pages viewed, referral source, device and browser information, IP address, and general location, and share it with Google. We also use HandL UTM Grabber, which records referral and campaign information (such as UTM parameters, gclid, landing page, original referrer, and IP address) with contact-form submissions. We do not allow advertising pixels or other tracking tools to disclose patient health information unlawfully. Listing a tool in this Policy or accepting a cookie banner does not authorize a disclosure of protected health information that requires a different legal basis.

If you choose to contact us, Gravity Forms processes and stores your submission for our practice, and our email delivery service sends the related notification. Our Patient Portal link takes you to a separate third-party portal governed by that provider’s privacy and security practices.
We use information to respond to inquiries and appointment requests, operate and improve the Site, understand Site performance and advertising, protect the Site, and meet legal obligations.
We may share information with service providers that help us operate the Site, forms, analytics, advertising, email delivery, and patient portal, only as appropriate for those services or as required by law. We do not sell personal information. We retain personal information as long as needed for the purpose for which it was collected or as required by law.
Nevada residents may have rights under Nevada law, including rights concerning the sale of certain covered information; we do not currently sell personal information as defined by Nevada law.
